T9o carry out needs -led assessments, as determined, of service users and/or their carers, to secure contributions to that assessment from them, other agencies as appropriate, giving full consideration to risk factors and their management.
Contribute towards promoting potential user and carer's ability to achieve or maintain their independence, recognising that userswish to continue, in the main, to live in their own homes wherever possible and practicable.
Ensure that all records and user information are maintained, using IT in accordance with the agreed data entry system.
To design tailored pacakages to meet assessed needs and to cost the care plan, assessing the user's ability to contribute towards the cost of services, in conjuction with administrative and other staff.
To priortise referrals and workloads.
To negotiate the provision of services at the most cost effective rate and ensure they meet service principles, objectives and specifications. To make imaginative use of community resources and stimulate the developements of local services.
To notify providers and quality controllers about deficiencies in the standards of services and inform service planners about unmet needs, gaps and shortfalls in provision
To monitor and review the care plan objectives, reassessig needs and revising the care plan accordingly
To contribute to the development of positive links with other departments of the authority, with the community as a whole and with partner agencies who are responsible for matters relating to both health and social care.
To liaise and work together with other colleagues and personnel in other agencies to identify concerns which may include adult protection issues and identify plans to address those concerns.
To be a flexible team member with a willingness to move between client specialisms and locations as circumstances in the department dictate.
To work within local protocols in relation to data collection for ongoing monitor of performance.
To undertake such training highlighted through the Personal Development Plan as required to achieve and maintain practice competence where required

Qualifications
Must have a relevent professional social Work qualification ie.. CQSW, DipSW, CSS or equivelant, ie RNMH RGN, RMN, Dip COT or other Professionals Allied to Medicine.
